MDS Coordinator

About this role:

Conduct the process for Long Term Care. This includes gathering information on the current residents/patients for future assessment, including physical and mental states. The process must follow OBRA (Omnibus Reconciliation Act of 1987) assessments to ensure that the facility complies with Medicaid and/or Medicare standards. Conduct new resident assessments, such as the Care Area Assessment (CAA), to help stabilize and improve the practices of the health care facility.  Will develop, implement, and evaluate individualized resident Care Plans.

Part-time, 24 hours per week (0.60 FTE), typically Monday – Wednesday, 8:00am – 5:00pm (may be times when assessment or transmission requires a different day).
An employee wanting to work full-time, (1.0 FTE) could work doing floor RN shifts.

What we are looking for:

  • Must possess a current RN license in the State of Montana
  • MDS Certification (obtained within 12 months of hire
  • 3 years LTC (Long Term Care) nursing experience
  • Must be able to lift up to fifty (50) lbs. and practice safe work habits
