Long Term Care – Care Coordinator/Social Services Designee
About this role:
Responsible for the coordination of care for designated patients/residents through the pre-admission stage to the discharge stage.
Plans, organizes, coordinates and directs the social services designee program in accordance with current federal, state, and local standards, guidelines, and regulations that govern such programs.
What we are looking for:
• Must possess an RN or LPN license in the state of Montana
• Obtain Social Services Designee Certification within three months of hire.
• Able to lift to fifty (50) lbs.
