Long Term Care – Care Coordinator/Social Services Designee
Description: Responsible for the coordination of care for designated patients/residents through the pre-admission stage to the discharge stage. Plans, organizes, coordinates and directs the social services designee program in accordance with current federal, state, and local standards, guidelines, and regulations that govern such programs.
Hours: Full-time; 36 hours per week (0.90 FTE); Benefit Eligible
Requirements: Must possess an RN or LPN license in the state of Montana AND be able to obtain Social Services Designee Certification within three months of hire. Must be able to lift to fifty (50) lbs. and practice safe work habits.
