To make our morning registration go smoother, we ask you register and pay for your team/individual on line.
Please complete registration form in it's entirety and indicate who you are paying for by checking the Paid Box next to the name. Please be sure to include handicap on form - if you have a MSGA handicap, they will be gathered the week prior to tournament, so no need to include.
$125.00 per person for "ALL IN" Games, Raffle etc.

Team Handicap based on 80% of Team Average
MSGA Handicaps will be used if one is not provided.

TWENTY-SIX YEARS of fundraising fun with our charity golf tournament! What better way to raise money for Sheridan Memorial than with a day of friendly competition!?
The Foundation's four person scramble on Saturday, July 15th, begins with registration at 9:30, followed by 10 o'clock tee time. On your way back to the club house stop in for a sloppy nacho lunch and finish off the 18 rounds relaxing on the course patio and having a steak dinner while winners are announced.

Breakfast: Juice, muffin.
Lunch: Taco/Nacho bar as you come by the Tee Box Tavern.
Dinner: Steaks, potato and garlic bread.
Only those who purchased the ALL IN for $125.00 will be able to participate in the course games--in addition they will have 50/50 and raffle tickets. Be sure to take the Grand Slam tickets with you out on the course as you will need them to participate in the games/give-a-ways.
Those who paid $75.00 registration will only qualify for the 4 hole prizes (longest putt, drive, closest to the pin and chip).
Longest putt, drive, closest to the pin and closest chip will be for front and back nine.
When you are done golfing we ask that you sign and have your score card attested by the opposite team and ONE person return BOTH cards to the OLD Pro-Shop for tallying.
ANNOUNCING OF WINNERS:
Winners will be announced for tourney, blind draw, hole prize, raffles, 50/50 etc. once all is tallied.
