It's a tradition of Sheridan Health Care Foundation to host their Annual Charitable Golf Tournament, and the tradition is well and alive again this year!

Please complete registration form in it's entirety and indicate who you are paying for by checking the Paid Box next to the name. Please be sure to include handicap on form - if you have a MSGA handicap, they will be gathered the week prior to tournament, so no need to include.

$125.00 per person for "ALL IN" Hole Prizes, Games, Mulligan, Blind Draw Calcutta, 50/50 etc.
Additional Dinners ($20.00)

Registration on Saturday morning opens at 9:00 am
Tee-off at 10:00 am

Men's Max Handicap is 30, Women's is 40
Team Handicap based on 80% of Team Average
MSGA Handicaps will be used if one is not provided.

TWENTY-SEVEN YEARS of fundraising fun with our charity golf tournament! What better way to raise money for Sheridan Memorial than with a day of friendly competition!?
The Foundation's four person scramble on Saturday, July 20th, begins with registration at 9:00, followed by 10 o'clock tee time. On your way back to the club house stop in for a nacho bar lunch and finish off the 18 rounds relaxing on the course patio and having a steak dinner while winners are announced.

MEALS:
Breakfast: Juice, Fruit & Pastries.
Lunch: Taco/Nacho bar as you come by the Tee Box Tavern.
Dinner: Steaks, Cold Salads and Garlic Bread.

COURSE GAMES:
Your $125.00 fee allows you to participate in the course games--in the spirit of the Summer Olympics join in on our themed games for chances to win prizes. These games are on the front nine only.

HOLE PRIZES:
Longest putt, drive, closest to the pin and closest chip will be for front and back nine.

FINAL TALLYING:
When you are done golfing we ask that you sign and have your score card attested by the opposite team and ONE person return BOTH cards to the OLD Pro-Shop for tallying.

ANNOUNCING OF WINNERS:
Winners will be announced for tourney, blind draw, hole prizes, raffles, 50/50 etc. once all is tallied.
