Due to the COVID 19/Corona Virus pandemic, Sheridan Memorial Hospital Association has activated its Incident Command with the purpose of preplanning for and mitigation of possible Corona Virus outbreaks in our community in the future weeks.COVID 19 Testing
Sheridan Memorial now has the capabilities of doing a rapid swab tests for SYMPTOMATIC patients only; however, a second test will be done at the same time to send off to the state to confirm the rapid swab results. We cannot do rapid swabs for asymptomatic patients. Monitor your phone calls and voicemail for someone to contact you with your results.COVID 19 (SARS Coronavirus-2) Testing Cost:

COVID 19 ANTI-BODY Testing
Sheridan Memorial now has the capabilities of doing a Direct Access Test for COVID-19 Antibodies. This is a test that requires NO provider order, will not be submitted to your insurance for reimbursement, will not be sent to your provider or part of your medical record and requires payment upfront.Testing will be done by appointment only by calling the Sheridan Memorial Laboratory at (406) 765-3727. Appointments will be available Monday through Thursday. Test results will take approximately 4 business days and will be mailed to you.

COVID 19 Vaccination
Injection Fee 1st Dose: $42.00Injection Fee 2nd Dose: $53.50
Billable to Insurance, self-pay will be billed to COVID 19 Uninsured Program.
Sheridan Memorial Hospital Association is currently collecting contact information for those who want to receive the COVID 19 vaccination. This includes ALL ages.
Sheridan Memorial offers the Moderna vaccine, thus only those 18 and older qualify for the vaccination. Sheridan Memorial may receive the 1-dose Johnson & Johnson Janssen vaccination, but there are no guarantees of receiving it as the vaccine is very limited.

You will be asked your first and last name, date of birth, if you have underlying health conditions, ethnicity and phone number to best reach you. Please be sure to monitor your messages/phone calls from Sheridan Memorial as we will call with a date/time for you to come in for your vaccination.

Items discussed and handed out to you at your visit:
- SMHA Vaccination Consent - Sheridan Memorial requires all who receive the vaccination to check off and sign the vaccination consent. The COVID 19 vaccine will not be administered if this consent form is not signed.
- imMTrax Consent - This consent form allows Sheridan Memorial to put your name and vaccination record in the Montana Immunization Information System. This will help you and your health care provider track the immunizations that you have received.
- Moderna Vaccine Fact Sheet - Sheridan Memorial is currently receiving the Moderna vaccine for its patients. Moderna is only recommended to those 18 and up. Be sure to consult with your provider on if the vaccine is a good option for you.
- Johnson & Johnson Janssen Vaccine Fact Sheet - Sheridan Memorial has LIMITED SUPPLY of the 1-dose Janssen COVID-19 Vaccine. It too is only offered to those 18 years of age and older. Be sure to consult with your provider on if the vaccine is a good option for you.
- V-Safe is a smartphone-based tool that will check in with you after your vaccination to check on symptoms from the vaccine.
