To begin the “Ideal You” process, you will first need to make an appointment to see the weight loss nurse for the initial assessment.  This will include body measurements, resting metabolic rate, vital signs, and completion of medical history form.  This appointment needs to be in the morning and you must be fasting, with no caffeine, no exercise, and no tobacco for the previous 4 hours.  You must have current labs, within the last 6 months and bring a copy with you.  If you do not have current labs, you can call the lab at 765-3726 to schedule a Community Health Panel which costs $35.  This blood work needs to be fasting as well.

After all the assessment information is obtained, you will then be scheduled for a complete physical and consultation with Jennifer Hoversland, FNP-C.  She will also review your results and further explain the weight loss options and which one is ideal for you.  From there, you get started onto a new healthier lifestyle!

Charges for all visits are due at the time of the visit.  A receipt will be given to you with all the information on it to submit these charges to your insurance company.  The physical exam, resting metabolic rate and consultation is $150.00, and may be covered by insurance companies.  If you choose the Ideal Protein Plan, the cost of the weekly food supplement is $90.  Our weekly coaching sessions are supportive and included in this charge.  Insurance companies more than likely will not cover the weekly food supplements.  Should you chose the Watch Weight Program, you will still need the initial testing, physical, and consultation.  After that, you will be charged approximately bi-weekly for office visits.  These office visits may also be submitted to your insurance company at a cost of $80.50.
