Passport Provider:
Sheridan Memorial is a Medicaid Passport Provider. Please contact your local Public Health Office to change your passport provider or for further information.

Our Services:

Sheridan Memorial Clinic takes appointments and walk-ins. Our three providers can treat acute medical illnesses and injuries as well as long-term care.
We have onsite Radiology and Laboratory services.
Family and Women's Health Care Services
Well Child visits
Sports Physicals/Camp Physicals are available by appointment only with an upfront fee of $25.00.
DOT Physicals are available by appointment only with an upfront fee of $60.00.
